Mail-in: legibly hand-print, on a 3″x5″ paper, your name, address, date of birth, phone number, and (optional) email address and mail it, along with a self-addressed stamped envelope (VT residents omit return postage), in a #10 envelope with proper postage affixed to:

Each mail-in entry must be submitted in a separate envelope, and must be received during the entry period dates to be entered into the applicable drawing.

Thirty-four (34) Prizes (one per day): LG G3 Beta unit mobile phone ARV: $650.00 As a condition of being awarded a prize, each winner must acknowledge and agree that: the prize is a pre-production “Beta” unit and is not a final commercial hardware/software; while not guaranteed, every effort will be made to update the prize phone to final commercial hardware/software when available (at Sponsor’s sole expense); and if selected as a winner, winner will not resell, offer at auction or give the prize phone away until after the phone is commercially available at T-Mobile retail locations.
