Free Codes:

Print your actual name and residential address (as they appear on your driver’s license) (No P.O. boxes), telephone number, date of birth and email address, on a 3×5 piece of paper and mail, in an envelope with proper postage affixed, along with a self addressed stamped envelope (Vermont residents may omit the self addressed stamped envelope) to the Free Code Address. Requests must be received by the Free Code Request Deadline. Each mail-in request must be mailed separately.
