Mail-in Entries must be postmarked during the Sweepstakes Period, and must be received no later than five (5) business days following the end of the Sweepstakes Period to be eligible.

We really appreciate your spending the time to answer the survey questions completely. Your full answers really are helping other Americans make better health care, test and treatment decisions.
The survey would ask a question like, what pregnancy symptoms did you have. I would check off the ones I had and then it would keep saying I needed to answer the other questions. If I checked off every single symptom on the list I kept getting the same message to answer all the other questions. This has got to be a scam.

The rules say you have to include a blank copy of the survey with your mail-in entry form but I can’t find out how to obtain a copy of the survey. Does anybody know?

I have tried taking the survey about 10 times even tried different browers to no avail…all I was able to get was a group of dots ….. so I mailed in, I was looking forward to taking the survey. The status of our health care concerns me
Same thing happened to me as happened to Crystal.. FYI, I experienced two miscarriages in the last 24 months (so…. severe fertility problems? and treated by a doctor). There should be a question under pregnancy to cover losses if you want to be a bit more comprehensive.
