Entries must be handwritten, legible and completed in full to be valid. Machine duplicated or reproduced entries will not be accepted. Only one individual’s name and e-mail address may be on each request and only one entry per envelope may be submitted. Must be postmarked by April 30, 2015 and received no later than 5 business days following the postmarked date to be included in the Promotion. Limit one entry per individual, per day.
