With Purchase Entry: clipping two UPC bar codes (no copies) from any two (varieties of specially marked MaMa Rosa’s Pizza and print on a 3″ x 5″ card the following information (please print clearly): your full name, address, city, state, zip code, signature, date, phone number and email address. Place the UPC’s and completed 3″ x 5″” card in envelope, affix postage and mail to:

Entry Limit: one entry per person, regardless of method of entry

One (1) Grand Prize: a trip for two to the Superbowl, on February 1st, 2015 in Phoenix, AZ, including tickets to the game. The prize includes a “meet and greet with Steve Young”, autographed Steve Young Jersey and football. Includes: round-trip coach air transportation for winner and one guest to Phoenix, Arizona from a major U.S. airport closest to the winner’s home as designated by Sponsor; double occupancy standard hotel accommodations for two nights; two tickets to the 2015 Superbowl on February 1, 2015 and certain ground transportation and meals in Phoenix as determined solely by Sponsor. Travel arrangements must be made through Sponsor’s agent on a carrier of Sponsor’s choice. ARV: $8,000
