Mail-In Entry: Obtain and completely fill out an official entry form found at all participating Guitar Center locations or legibly hand-print your first and last name, address (including zip code), age, and day and evening phone numbers on a 3″ x 5″ card and then mail the completed entry form or the card in a stamped envelope to:
