Mail-in entry: Complete the Official Entry Form in the direct mail piece or an Official Entry Form available (while supplies last) at select retail locations by confirming or hand-printing your name, address, birth date (required only for Official Entry Forms available at select retail locations) and telephone number and signing your full legal name to certify that you are a smoker, 21 years of age or older, and send it to the address indicated. The Official Entry Form also contains five (5) survey questions. While it is requested that you answer all five (5) survey questions, answers to the questions are not required in order to enter the Sweepstakes and will not affect your chances of winning. In addition, if you choose to answer the questions, the content of your answers will not affect your ability to enter the Sweepstakes or chances of winning. Mail-in entries must be postmarked by October 31, 2014, and received by November 7, 2014.
