Official Entry Form: Visit the Simon Guest Service desk at a participating Simon mall location, or various events at the participating Simon mall to request an official entry form. Complete the entry form including your name and email address. Place the form in the designated receptacle, or hand it to a Simon mall representative. Where available, you may complete and submit the form on a tablet computer. You will receive one Sweepstakes entry.
Entry Limit: two entries per person by a combination of methods described above.
