Eligibility: offered only to individuals who are legal residents in the fifty (50) United States and the District of Columbia, and are at least 18 years old, who have internet access and a valid email address at the time of participation in the Promotion. Television Internet access is not valid.
Entry Dates: The Instant Win Game begins on June 19, 2009 and ends on July 10, 2009. The entire promotion including the sweepstakes ends on December 31, 2009.
How to enter: Visit www.LaVicRewards.com. The initial login in any 24 our period during the Promotion Period will constitute an entry in the Instant Win Game. For the purposes of the Promotion, a day begins at 12:00:01 a.m. PT, and ends at 11:59:59 p.m. PT. You can skip the survey.
Account Limit: Only one (1) password may be used by any participant and there is a limit of one (1) Account per person/Account/email address/street address or household. Should any participant attempt to use more then one (1) password or establish more than one (1) Account, that participant may be disqualified.
There are no codes needed to play the instant win game. There are codes needed for the sweepstakes.

We have had no problem using codes with the : in them. Make sure you are putting them in the right place. There is a place for 14 digit codes and another box for putting 18 digit codes like the one you mentioned.

When I log in, it brings me straight to the place where you enter codes. Is it supposed to tell me if I won or lost the IWG?
It's supposed to take you to the survey page. Scroll to the bottom and click Skip the Survey. If it doesn't work right then try deleting the cookie for that site and try again.

This promotion is one of the best I've seen in a while. I did have some trouble printing the coupons. The first two printed without an issue. I properly downloaded and installed the coupon printing program yet still had trouble. When the print window instructs you to close the window, Don't unless it actually prints. Click the help link and complete the form. At that point, the coupon will be mailed to you.
