1. Health Facts 1 2 3: take a simple, 3 question multiple choice (or true/false) quiz about how to manage health care costs.

2. Look and Learn: watch a short video about how to avoid cost surprises after enrolling in a health care plan. After watching it, you must answer one question that pertains to the video.

Tip: your questions will appear 30 seconds after the video has started.

UnitedHealthcare Perfect Match Quiz

3. Perfect Match: play a memory/concentration game with generic health care terms that will help you understand the basic cost terminology that you may come across in your health care plan.

Entry Limit: one entry per person per day per quiz + bonus entries. Weekly winners remain eligible to win the Grand Prize.
