As long as it is legitimate paperwork coming from that company then yes, you should fill it out and fax it in. Every company does things differently and this is probably what they require.

I received an email saying I won an ipod shuffle. Then it asked me to fill out a form and fax it to a certain number. Is this normal for sweepstakes winners? It just asks for name, birthday, address and telephone number. But I wanted to double check before faxing anything.

Sure, why not. You could enter any kid sweepstakes for your grandchildren as long as they are the appropriate age. Just use your daughter or son's name, address, and email as the parent/guardian unless you are their guardian.
